Form Replies are available in the "Manage Forms" section of Communicate for E-Board and above. 

  • Communicate>Forms>Manage Forms


Form replies can be sent to individuals (any Member of E-Board or Important References) or a group (Admin, E-Board etc.) 

  • Replies sent to individuals (Chapter President, VP CRS etc.) will have a copy of the form reply emailed to them. This is especially helpful for ongoing forms such as a Check Requisition Form 
  • Replies sent to a group (E-Board) will need to log into MCR on a computer to view the replies in Manage Forms. 


To view a form reply you will click on the "Report" icon in "Manage Forms" 


You will be brought to a screen that will show you: 

  • Who has/has not submitted 
  • For ongoing forms you will be able to see each submission a Member has submitted 
  • You will be able to see groups required to fill out the form and who can view replies 
  • For single sign forms: 
    • You will be able to see how many outstanding submissions there are 
    • How many forms have been submitted 
    • Who has submitted and still needs to submit
    • The due date for the form 


  • Form submissions will be held in MCR so there is no need to print each submission. 
  • Even once a form has been archived, you can go back and view submissions. 
  • In the event you need to print a record of a form submission for a third party, such as your Fraternity & Sorority Life office or National Headquarters, you can do this by simply clicking the printer icon next to the submission



  • This will open the form in it's own tab 
  • Click the printer icon on the opened form to save as PDF to send electronically or to print a paper copy 



  • The print options screen will open, you will either choose save as PDF or your printer in the destination line depending on whether you want to print a physical copy or save a PDF to send electronically
