To create a new form, go to My Access > Eboard > Forms Designer

  • If there is a form already listed that you just want to recreate you can click on the "+" icon in the "copy?" column next to that form.  This will allow you to keep the same format and just change the questions/details without having to completely reinvent the wheel.  


  • To create a completely new form click "Create New Form" button 
    • Give the form a name and a heading (these can be identical) 
    • You can add optional text in the leading paragraph if you would like 
    • If you want members to be able to fill out the form more than once check the box for "multiple replies" 
    • Select the group(s) of members you want to fill out the form 
    • If you want replies to remain anonymous check the box for "anonymous replies" 
    • Choose who you want to see replies - admins or eboard members 
      • if you want a certain member to receive MCR mail every time a reply is submitted, check the "Position/Member List for MCR mails" and then select the member(s) from the drop downs
    • DO NOT check the "published" or "archived" boxes at this point - if you do and then save your form you will NOT be able to edit it.
    • Begin adding your form fields (you can add 5 at a time)
      • Number each one as you want it to appear
      • Select the type of field (free text, drop down, check box, etc.) 
      • Check the box if you want to make that question/item mandatory to answer
      • Give it title/label (example:  First and Last Name; Year in School) 
      • Select the location of the label 
    • Save Changes once done
      • You can then review your form and make adjustments if necessary 
      • If you needed to add more than 5 fields you can continue adding 5 additional ones. 
    • If your form looks they way you want it to, scroll down to the bottom of the "Edit Main Form Details" section and check the "Published" box.  
      • This will allow cause the form to show up under Misc > Polls & Forms > Forms for your members to see.  
      • Warning:  Once you publish your form you can no longer edit or delete it.
